
Children's Health Home Care Management
Health Home Care Management is a free service for people with active Medicaid that meet eligibility requirements based on their physical and mental health conditions.

What is Health Home Care Management?
A Care Manager will work with your child and family to build on your strengths and develop skills to address any concerns for your child’s well-being. Care Managers work with the child, family, and all other people involved in child’s care (pediatrician, dentist, counselor, and school staff) to create a care plan where everyone is working on a common goal. If additional supports are needed, your Care Manager will help to connect you with appropriate services to meet your child’s needs. Care Management services are family-driven, youth-guided, and person-centered to meet each child’s unique situation.

A Care Manager will provide:

Comprehensive Care Management: Support for all aspects of your child’s well-being, in partnership with family and care team.

Care Coordination and Health Promotion: Working with current providers and scheduling appointments to promote physical and mental health, as well as assisting with finding new providers.

Comprehensive Transitional Care: Assistance following ER visits/hospitalizations to assist with follow up appointments.

Individual and Family Support: In-person visits are required for the program and Care Managers are available by phone after hours for crisis/emergency calls.

Referrals to Community and Social Support Services: Will link child with appropriate services to improve their overall well-being.

Help managing all types of care, including: medical, counseling, and social services. Will ensure child is linked with a primary care physician and dental provider, counseling and medication management services when needed, attend school meetings and assist with advocacy, assist with housing and transportation needs, and link with other services as needed.
